Personal Information Collection Statement

(Applicable to Membership Registration Application)

This Personal Information Collection Statement (the "Statement") is prepared in accordance with the Hong Kong Personal Data (Privacy) Ordinance (Cap. 486) and the guidelines issued by the Office of the Privacy Commissioner for Personal Data (PCPD). The Association (i.e., "China Paragliding Association Limited", hereinafter referred to as "the Association") respects your privacy and is committed to collecting, using, and protecting your personal data in a lawful, fair, and transparent manner.

Definition of "Personal Data": According to Section 2 of the Personal Data (Privacy) Ordinance, "personal data" means data relating to a living individual who can be directly or indirectly identified, and which is recorded in a form in which access or processing is practicable.

1. Purposes of Collecting Personal Data

The Association collects your personal data primarily for the following purposes:

• Processing your membership application and issuing your membership card;

• Arranging member activities, training, competitions, and issuing notifications;

• Sending newsletters, event information, safety reminders, and emergency notices;

• Processing membership fee payments and maintaining invoice records;

• Conducting internal statistical analysis, membership service evaluation, and policy review;

• With your consent, sending you promotional information from the Association or its partners (e.g., equipment discounts, overseas flying trips, sponsor events, etc.);

• Complying with relevant legal or regulatory requirements (e.g., insurance, safety regulations, government statistics, etc.).

2. What Personal Data Will Be Collected?

You may be required to provide the following information, including but not limited to (fields marked with * are mandatory):

• Name (Chinese/English)*

• Gender*

• Date of Birth*

• Contact Phone Number*

• Email Address*

• Correspondence Address*

• Emergency Contact Name and Phone Number*

• Paragliding Qualification Certificate Number (if applicable)*

• Relevant Equipment Information

• Past Flying Experience or Training Records (if applicable)

• Bank Account or Credit Card Information (for payment purposes only; optional)

• Photograph (for membership card; optional)

• Copy of Hong Kong Identity Card (for Sporting License Application)  

To assist members in applying for an Sporting License, we may request a copy of your Hong Kong Identity Card. For privacy protection, the last four digits of the HKID number must be redacted. This practice aligns with the Office of the Privacy Commissioner for Personal Data’s guidelines on handling HKID numbers.  

  

The HKID copy will be used solely for:  

 - Verifying your identity;  

 - Submitting your application to relevant sports bodies;  

 - Internal record-keeping to comply with immigration or sports federation requirements.  

  

We will not share your HKID copy with any third party for marketing or other unrelated purposes unless you explicitly consent or required by law.  

  

The HKID copy will be retained for no more than 12 months after the sporting license application is completed, then securely destroyed..  

  

You must confirm your understanding and consent to this use. If you do not provide the HKID copy, we cannot assist with your Sporting License application.

3. Use and Transfer of Personal Data

3.1 Scope of Use

Your personal data will only be used for the purposes stated above and will be processed by authorized staff of the Association and partners (e.g., event venues, insurance companies, technical service providers) on a "need to know" basis.

3.2 Transfer to Third Parties

The Association may transfer your data to the following third parties:

• Event planning companies and venue management organizations cooperating with the Association;

• Insurance companies (for the purpose of arranging insurance coverage for members);

• Cloud service providers or data processors (for membership system management);

• Government or regulatory authorities (if required by law);

• Other third parties with your consent.

3.3 Special Handling of Identity Card Copy

The copy of the Identity Card collected by the Association will be used solely for the purpose of applying for the Sporting License and will not be used for any other purposes (e.g., direct marketing, promotion, statistical analysis, etc.). All copies of the Identity Card will have the last four digits redacted and will be stored in an encrypted server or a locked filing cabinet, accessible only to the "Athlete Card Application Designated Personnel".

If a member wishes to withdraw their consent or request the deletion of their Identity Card copy in the future, they may email the Association at secretary@hkpa.net. The Association will complete the processing within 30 days of receiving the notice.

All third parties are required to comply with a confidentiality agreement and may only use the data for the purposes specified by the Association.

4. Data Retention Period

Your personal data will be deleted within 12 months after you withdraw from membership or cease participating in the Association's activities, unless:

 Legal requirements mandate a longer retention period (e.g., insurance claim records);

 The Association needs to retain the data to fulfill contractual or legal obligations;

 You have consented to the retention of your data for research or statistical analysis (such consent may be withdrawn at any time).

5. Your Rights

Under the Personal Data (Privacy) Ordinance, you have the following rights:

• Right of Access: You may request access to your personal data held by the Association.

• Right to Rectification: You may request correction of inaccurate or outdated data.

• Right to Withdraw Consent: You may withdraw your consent to promotional information or thirdparty transfers at any time.

• Right to Complain: If you have concerns about the Association's handling of your personal data, you may lodge a complaint with the Office of the Privacy Commissioner for Personal Data.

For complaints or requests to amend your data, please email secretary@hkpa.net. The Association will process your request within 30 days of receiving the notice. If you make a data access or correction request, please provide the following information for identity verification:

 Name and contact phone number;

 Membership number.

The Association may charge a reasonable administrative fee (e.g., photocopying costs) that is not excessive for access requests, but no fee will be charged for correction requests.

6. Consent Mechanism (Important)

Please tick the following options during registration to indicate that you have read and agree to this Statement:

☐ I agree to the Association collecting and using my personal data for the purposes stated above.

☐ I agree to the Association collecting and using a copy of my Hong Kong Identity Card (with the last four digits redacted) for the purpose of assisting with my Sporting License application. (If this box is not ticked, the Association will be unable to assist with your Sporting License application.)

You may withdraw your consent to promotional information or third party transfers at any time by emailing secretary@hkpa.net. The Association will complete the processing within 30 days of receiving the notice.

Note: If you do not tick the "Agree" box, the Association may be unable to process your membership application or provide related services.

7. Data Security Measures

The Association has taken the following measures to protect your personal data:

• Restricting staff access permissions to only necessary personnel;

• Personal data will be stored on online servers with security systems provided by third parties.

8. Data Breach Notification Mechanism

In the event of a personal data breach, the Association will notify affected individuals within a reasonably practicable time.

（適用於會員登記申請）

本《個人資料收集聲明》（「聲明」）根據香港《個人資料（私隱）條例》（第486章）及個人資料私隱專員公署（PCPD）發出的指引編訂。本會（即「中國滑翔傘協會有限公司」，以下簡稱「本會」）尊重您的個人資料私隱，並承諾以合法、公平、透明方式收集、使用及保護您的個人資料。

「個人資料」定義: 根據《個人資料（私隱）條例》第2條，『個人資料』指與一名在世人士有關、可直接或間接確定其身份、且以可供查閱及處理形式記錄的資料。

一、收集個人資料的目的

本會收集您的個人資料，主要用於以下目的：

• 處理您的會員申請及發放會員證；

• 安排會員活動、訓練、比賽及通知；

• 發送會訊、活動資訊、安全提示及緊急通知；

• 處理會費繳交及發票紀錄；

• 進行內部統計分析、會員服務評估及政策檢討；

• 如您同意，可向您發送本會或合作夥伴的促銷資訊（如裝備優惠、海外飛行團、贊助商活動等）；

• 遵守相關法例或監管要求（如保險、安全規範、政府統計等）。

二、將會收集哪些個人資料？

您可能需要提供以下資料但不限於（*號為必填欄位）：

• 姓名（中文／英文）*

• 性別*

• 出生日期*

• 聯絡電話*

• 電郵地址*

• 通訊地址*

• 緊急聯絡人姓名及電話*

• 滑翔傘資格證書編號（如適用）*

• 相關設備資料

• 過往飛行經驗或訓練紀錄（如適用）

• 銀行戶口或信用卡資料（僅用於繳費，非必填）

• 照片（用於會員證，可選擇不提供）

• 身份證副本（用於申請體育執照）  

為協助會員申請體育執照，本會會要求會員提供香港身份證副本，惟為保障會員私隱，身份證號碼之最後四個數字必須被遮蓋，以減少誤用或濫用風險。此安排符合香港個人資料私隱專員公署《身份證號碼及其他身份代號實務守則》之建議。  

  

本會只會將身份證副本用於以下目的：  

 - 核實會員身份；  

 - 向相關體育機構提交申請體育執照；  

 - 作為本會內部記錄，以符合《入境條例》或體育總會之要求。  

 

本會不會將身份證副本轉交予任何第三方作促銷、推廣或其他非申請體育執照之用途，除非事先取得會員明確同意或法律另有規定。  

  

身份證副本將於體育執照申請完成後保留不多於12個月，期滿後將以安全方式銷毀，並不會用作其他用途。  

  

會員須確認知悉本會收集及使用其身份證副本之目的，並同意本會按上述用途處理其個人資料。如會員不願提供身份證副本，本會將無法協助其申請體育執照。

三、個人資料的使用與轉移

1. 使用範圍

您的個人資料僅限於上述目的使用，並由本會授權職員及合作夥伴（如活動場地、保險公司、技術服務商）在「必要知悉」基礎上處理。

• 本會合作的活動策劃公司、場地管理機構；

• 保險公司（用於為會員投保）；

• 雲端服務商或資料處理商（用於會員系統管理）；

• 政府或監管機構（如法例要求）；

本會收集之身份證副本，僅限用於申請體育執照之目的，並不會用作其他用途（例如：直接促銷、推廣、統計分析等）。所有身份證副本均會被遮蓋最後四個數字，並儲存於加密伺服器或上鎖文件櫃，僅限「體育執照申請專責人員」存取。  

  

如會員日後欲撤回同意或要求刪除身份證副本，可電郵本會 secretary@hkpa.net，本會將於收到通知後30日內完成處理。

所有第三方均須遵守保密協議，且僅可為本會指定目的使用資料。

四、資料保留期限

您的個人資料將於您退會或不再參與本會活動後12個月內刪除，除非：

• 法律要求保留更長時間（如保險索償紀錄）；

• 本會需保留資料以履行合約或法定責任；

• 您已同意保留用於研究或統計分析（可隨時撤回同意）。

五、您的權利

根據《個人資料（私隱）條例》，您享有以下權利：

• 查閱權：可要求查閱本會持有的您的個人資料；

• 更正權：可要求更正不準確或過時的資料；

• 撤回同意權：可隨時撤回對促銷資訊或第三方轉移的同意；

• 投訴權：如對本會處理個人資料有疑問，可向個人資料私隱專員公署投訴。

如有投訴或更改要求，可電郵至secretary@hkpa.net，本會在收到通知後30日內處理。如您提出查閱或更正要求，請提供以下資料以供核實身份：

• 姓名及聯絡電話；

• 會員編號；

本會可能就查閱要求收取不超乎適度的行政費用（如影印費），但不會就更正要求收取費用。

六、同意機制（重要）

請於登記時勾選以下選項，以表示您已閱讀本聲明並同意：

☐我同意本會根據上述目的收集及使用我的個人資料。

☐我同意接收本會發送的活動推廣、優惠資訊及合作夥伴訊息（可隨時退訂)

☐我同意本會收集及使用我的身份證副本（遮蓋最後四個數字）以協助申請體育執照。（如不勾選，本會將無法協助申請體育執照）

您可隨時透過電郵secretary@hkpa.net，撤回對促銷資訊或第三方轉移的同意。本會將於收到通知後30日內完成處理。

注意：

如您未勾選「同意」，本會可能無法處理您的會員申請或提供相關服務。

七、資料保安措施

本會已採取以下措施保護您的個人資料：

• 限制員工存取權限（僅限必要職員）；

• 個人資料會儲存在由第三方提供有保安系統的網上服務器中。
